Christmas Stocking Initiative

Dear Caseworker, This form is to request a holiday stocking for children on your caseload. This can be children in custody, or children in vulnerable situations who you feel could use some extra joy! Our only requirement is that you are currently working with the family and anticipate that you still will be by the end of December. We are so excited to engage our community in this initiative over the holiday season! Please help us stay organized by submitting a separate form for each household. You may add multiple children in the same household on the same form. Every household will be assigned a number. Initials will not be shared with sponsors in the community. They are simply for your reference when receiving filled stockings back for distribution. (which will happen on December 15 at your staff meeting). Requests will close on November 16th. Thank you!
